Communication, it just does not happen within our NHS, after combined assessment and being moved to a ward, in combined assessment they know that the patient is vegetarian, needs tablets crushed, needs high risk mattress. On thickened fluids, nurses need training, stage two fluid, seen it so thick you could cut it with a knife. Cot sides are up for a reason and when loved ones visit and both cot sides are down, feet on a pillow to protect heels but patients own repose boots are on the window ledge. Ask to see a nurse in charge and on occasions that doesn't happen. I'm told hospital is the best place to be when ill but when the family gets that feeling that the person is at risk then something is badly wrong
