"Constant problems with prescriptions"

I have multiple repeat prescriptions, and unfortunately since I've moved to this surgery I am having to chase or correct something at least once a month. 

I'm currently chasing something I ordered a week ago that hasn't even been sent to a pharmacist yet. I spoke to a receptionist on Monday as they apparently were using my old address (despite me having given my address three times - twice by email with proof of address, and once going into the surgery) and the issue seemed to be resolved but when I looked at the app to see why I hadn't had a notification from the pharmacy yet it had been rejected. I've emailed and tried reordering through the app but I have heard nothing and will now run out of this medication. The last thing I ordered I had to chase for two weeks and was sent to the wrong address. 

I've had things sent to the wrong address, which was stolen by someone potentially quite dangerous who now has my full name and was very aggressive when I tried to get it back and has turned up at my house. I have had things sent to my old address, to the wrong pharmacy, I've been told its at a pharmacy but when I arrive they've not been sent it, I dread requesting my medication because I know this is going to happen and I'm constantly worried about running out. 

The staff at the surgery are helpful when I do speak to them but there is clearly something going wrong in the system and there has been no sign that anything will be done to prevent it happening over and over again. I've even tried speaking to the people at the prescription app but they just tell me to speak to the surgery, but when I mention it at the surgery I only get a solution for the particular prescription, never the ongoing issue.
