"Supportive medically and administratively, but..."

I read the following:

If you have been advised by the surgery that your medication review is due, please use our Medication Review form.

However, when I click on 'Medication review Form' I read:

'How embarrassing, this can not be found'.

So: how do I in fact apply for a medication review online ?

In every other respect I consider myself and my wife lucky to be patients of Manor Farm - medical treatment, administration, dispensary, are all competently and considerately and cheerfully carried out.
