"Care at home from Urgent Community Response team"

About: Urgent Community Response Mid Hants

(as a staff member posting for a patient/service user),

I was unwell at home and the Urgent Community Care Team came to visit me at home when I was unwell. It was always reassuring seeing the team and I was pleased that I could rely on this service to give me assurance when I wasn't feeling well.

I am now feeling better and I was pleased to have the team coming in. I wasn't always sure at the start what they would be doing and when I was unwell and need to go to hospital for a day it wasn't really explained to me why.

I do appreciate that the team kept me well and at home and that my trip to hospital was only a short one. The team helped when I wasn't sure which tablets to take when I came home. I now feel settled and recovering.

I am pleased that the team reassured you and I will of course pass that on. I hope the reliability of the team also helped your recovery.

I would like to help with the way you felt about communication. I think sometimes as busy professionals we assume too much. I would like to share your comments with the team and remind them to always explain everything including why we are visiting. This can get lost in a busy day but it is so important to make sure we communicate well. We are also developing a patient and carer leaflet for us to leave in our service users homes.
