I attended ENT yesterday morning then had an appointment at Orthopaedics in the afternoon. Orthopaedics couldn’t find my notes so I told them I’d been at ENT in the morning so instead of the nurses had to go and find them. Surely there is  a system for notes if a patient has more than one appointment in one day.

I am sorry to hear of your experience in the hospital today with missing notes. There is a process in place for notes when patients are attending 2 clinics on the same day, unfortunately this appears to have not been followed on this occasion.

I will personally investigate this and make sure all staff are aware and follow the procedure in future. Thank you for bringing this to our attention.
