"Extremely Disappointed "

I had to register with my GP so i came in mid December to fill the form and submit it, after submitting it, they checked my passport ID and i waited there for half an hour, after two days, I went there to ask if i am registered or not, they told me that we don’t have your form and record. My form was disappeared and they didn’t have any clue, BTW I submitted another form and they asked about my date of birth three times, now about a week later I received “Confirmation of registration form” and when I checked that, my date of birth was written wrong..Now what am I supposed to do..tired of filling forms and going there
