A wonderful service- hoping this service is retained! 

I had input from the OT and Pharmacist- in particular, the Pharmacist Carla  gave good advice.

Thank you for your kind response. I agree the service is very beneficial in order to keep patients at home. Carla the pharmacist plays a vital role within the team and completes a full medicine assessment on all patients. I will pass on your comments to the team in particular the OT and Carla.
