"Service after bereavement"

Health care service during stay within hospital was good however upon receiving the news of the passing of my mother on a Friday night, I have had to chase to receive notification on death certificate.   Albeit over a weekend and a public holiday, service resumed to normal on the Tuesday and find myself after being assured by ward consultant that death certificate would be processed as a matter of urgency.   I have phoned the registrar office for deaths and still nothing has been received.   I am finding the level of service to be inefficient as I can't proceed with arrangements.

Thanks for letter with the outcome of my complaint regarding delay and poor communication of obtaining the death certificate for my late mother.

I feel there is nothing to achieve in pursuing this complaint but find it hard to acknowledge that all medical ward team were unavailable for 5 days, also how low prioritised the completing of the death certificate was, also the unavailability of admin staff in a hospital on a Wednesday when the hospital has admin cover 24/7.

Although I'm accept closure of complaint, I still think being a University Teaching Hospital that the issue should be raised with the Quality Improvement Team to review and hopefully produce a more efficient Death Certification pathway to reduce further anguish on bereaved families.
