"My Mother"

About: Enhanced Care / Acute Care at Home

(as a carer),

On 7th March 2021 my mother became very short of breath and was clearly unwell.  We contacted the Out of Hours Doctor who came to the home to see Mum.  My Mother was adamant she  did not want to go into hospital as she had been in hospital for 5 weeks over Christmas and due to Covid she was not allowed visitors, which of course was an extremely difficult time for both her and her family. 

The Doctor said he would contact her GP and suggested the Acute Care Team should come to the home to see her.  The Acute Care Team arrived to access Mum on the 8th March.  What a relief it was for us to have this Medical Support as since Mum came home from hospital we had only received phone calls from her GP which was totally inadequate.   

We had an extremely professional Medical Army ascending on her home.   They completely took over her care.  We had Consultants, Doctors, Nurses, Dietitians, and Occupational Health etc. all working tirelessly in the home to stabilize her.  They carried out tests and prescribed medications and continued to work with her for almost a week until her condition improved.  The care she received from the Acute Care Team was second to none.  We believe she would not have been given the same one to one care in hospital and of course she was able to remain in her home with her family around her.

We as a family are always be grateful to the whole team and would highly recommend this brilliant service.

It is a particularly difficult time for the families of care home residents, so we really value you taking time to feedback to us.

If suitable for our service, quite often the outcomes are better for patients if they remain in their own residence and I am so pleased to hear that this was the case for your mother.

I am so reassured that you have likened the team to a 'medical army' who worked diligently to stabilize your mum with success and I sincerely hope she remains well.
