(as a relative),

Fathers health deteriorated had chronic heart failure and kidneys were failing too.  He didnt want to be admitted to hospital. GP referred him to team at RVH and very next day a care plan was devised and hospital bed delivery arranged. The lead nurse practitioner - Jane was excellent in keeping in touch with family and explaining everything she was putting in place.  Thanks to Jane and the team he was able to spend his last days at home surrounded by family .
