"End of Life Care"

Having just said a final farewell to my mother, we wanted to write an appreciation of the care and support we received.  The team at our medical practice, the Medical and District Nurse Team by their care, swift decision making and understanding, supported our request to keep her at home for the care she needed.  On going support during the period we were caring for Mum guided our approach and reassured us.   We received timely, effective and reassuring care from the Forth Valley Palliative care team.  Our local pharmacy were efficient and caring and dove tailed their service with the local medical support. 

To be able to care for Mum at home was such a gift, and we are very thankful to the teams who made this possible.
