"Covid questionnaire"

About: Perth Royal Infirmary Perth Royal Infirmary / Urology

(as a service user),

I was requested to complete to complete a COVID questionnaire prior to my appointment. However either I failed to understand the web site, it was not working or totally user unfriendly.

In short it didn't work for me.

Dear Muir,

Thank you for bringing this to our attention, I am sorry this was your experience. At the moment I am trying to ascertain with colleagues why this problem occurred and what we can do to rectify it. I also note your comments about the questionnaire not being user friendly so we will look in to this too. If you have any specific views or ideas on how we could improve the questionnaire we'd be really keen to hear them.
