"Mistakes made by office staff"

(as a parent/guardian),

After waiting 3 weeks for an appointment, collecting the forms from my GP prior, we got there, waited 40 minutes and found the gp's staff had given us the wrong form. On phoning, the response was 'mistakes are made'. Honestly, it doesn't take much to print out 2 separate forms and check them, this isn't a mistake so much as bad practice. Not the first mistake we gave dealt with, yet the onus is on me to make another appointment (weeks away), to collect more forms, pay for yet another carer while we attend this unnecessary appointment.
