"Domiciliary care and associated community services"

About: North Down Community Services North Down Community Services / Enhanced Care At Home

(as a relative),

Following a recent hospital discharge my Granda was being cared for by the Trust's North Down and Ards domiciliary care department. The care he received was second to none.

The care staff and team co-ordinators escalated concerns to his care manager, Anna-Maria. Consequently, District Nursing and The Enhanced Care at Home (ECAH) became involved with his treatment. I was so impressed with the communication and co-ordination between the different services. As a result Granda received the care he needed at home, without delay and avoided a lengthy hospital admission. I am very grateful to Anna-Maria and all of the staff and services involved.

thank you for taking the time to share your Granda's experience.

Our team work so hard to support everyone we can to prevent a hospital admission or enable a speedy discharge from hospital, they really value and appreciate all comments for us to improve what we do. It is so important that we work with all our colleagues in different services to make this happen.

It is a joy to hear when things go so well. I have shared your story with the team which they truly appreciate.

I hope your Granda is keeping well now and wish you all the best and hope you and your Granda are able to see each other in the coming days and have a good Christmas.
