I have sent a PALS communication(s) relating to both my mum's stay and circumstances of her discharge from the Oncology ward -
My concerns are many:
The apparent lack of basic care and compassion at Queens. The basic nursing and doctor care. How my mum was not even fed appropriately or sufficiently with poor quality food.
How was my mum discharged considering her poor health. Psychosis was subsequently written in her discharge notes. A psychologist called my mum at home after her discharge. My mum refused the call. No information was provided why her mental health was not supported/assessed whilst my mum was in hospital.
Neither myself or my dad were informed or consulted of/on her general health status on her discharge. Neither was my dad supported to adequately care for her. My mum informed him that the message she received from the medical team was that she'd be walking in 4 months. The discharge notes provided no prognostic information.
My dad called the hospital numerous times following her discharge. This included to discuss my mum's Occupational support as well as ongoing CLL support. This is what triggered her discharge notes.
My mum refused to go back to Queens due to her trauma associated with her stay. Myself and my dad believing that my mum was in rehabilitation managed to obtain a bed at Basildon rehabilitation unit. Again, not communicated to us my mum was deteriorating and passed away on 28 Oct 2020. We were never involved in any conversations relating to her discharge or subsequent withdrawal of medications or options to be cared for at home. All discussions with Occupational Health were around rehabilitation at home. We are not sure what information was communicated between Queens and Basildon.
Communication with a crisis unit was triggered post Queens but with no positive outcome. Only a letter that transferred responsibility to Basildon.
I do not understand how the sequence of events could have happened. As a family member i was unable to go into the hospital and speak to staff. No mechanisms appeared to be in place to provide sufficient information to support close family to support the patient and plan accordingly. I was unable to be with and support my mum in her final days.
