Inconsistent care.

Regardless of your condition and pleading once they have decided to did-charge regardless of whether you have any further care or support. You discharged and indeed blamed for advising them if your distress or doing things within their time frame which for me was registering with a new GP. Medication was not provided when requested even though it was offered previously. I was left to my own devices it was an awful and highly disturbing situation.

Thank you for this feedback. We are sorry that you felt you were not ready to be discharged from the Home Treatment Team. And that you felt follow up support wasn't in place. With regards to the service – we often say the purpose of the team is to offer intensive but brief support whilst you are at home/ in the community to help manage a mental health crisis without the person needing to be admitted to hospital, or support people come out of hospital earlier. We endeavour to discuss and agree further and ongoing support available in the community when the service stops. It is difficult to advise you here without knowing your particular circumstances. Moreover we respect your privacy. So can we ask you to contact the Home Treatment Team for a proper discussion about the next steps in your care so we can ensure you have the contact numbers for the team/person providing ongoing care, and make sure you have your prescribed medication (if any medications were recommended). We are so sorry that you felt somewhat abandoned. We want to remedy this and ensure that you have the necessary support going forward, so we look forward to hearing from you.
