(as a service user),

I am due to have a telephone appointment regarding a bladder problem and have received a form to complete prior to the appointment asking for feedback and information.   This form has got me spitting feathers, made me angry and has blown my life apart - it is so emotionally distressing.   The nurse calling should have read my notes already and have the background information relevant to my medical issue.   Why are they asking how much my house is worth?   Why do they need to know the height of my bed?   Why is my mental health brought into question about a bladder condition?   It is embarrassing enough without having to speak about things like this over what I feel is an insecure medium - the telephone call could be hacked, recorded etc.   I have been told this appointment will last between 1 hour and 1 hour 30 minutes - is this because of the non-relevant questions.   I am constantly worrying about this appointment now.   I just want help with my medical problem not the third degree! !   If I have other medical issues I am quite competent to seek help out myself! !

I was so sorry to read about your experience with the form you have been asked to complete prior to the appointment asking for feedback and information. I would like to make sure that your feedback gets to the right staff to help and to apologise also for the distress it has caused you.

We don't want anyone to worry about appointments. Please be reassured, the help with your medical problem is the main focus of the reply.

I can't see from your feedback which service or team you have received the referral from?

I can contact the team to share your feedback if you want them to contact you?

Any suggestions for improvement to our forms and services are really valued.

Hi Jane

I am getting rather upset at the nature of the questions I will be asked at my telephone assessment. It would seem their isn't an area of my life the nurse isn't going to ask about.I have lost sleep and feel degraded and humiliated at the very invasive questions that might be asked.

I feel like my whole life us being torn apart. Something my husband wasn't put through during his cancer diagnosis and treatment. Yet I ask for help with my bladder and u get the 3rd degree.

I was referred by my drs surgery and I am under nhs nottinghamshire healthcare nhs foundation trust. My appointment is concerning bladder issues.

I feel it is unnecessary for me to relive past medical issues some of which are very embarrassing and others very distressing. Particularly as the nurse will be able to see my medical notes. This is even more of a concern as the phone call isn't a secure.

I am also concerned that I will be made to wait several weeks if not months for a physical examination. As the problem is a physical issue I can't see how it can be properly assessed by phone. I am keen to have any diagnostic tests carried out as quickly as possible.

The issue is really getting me down with the 5 week wait till my appointment not helping.
