"Missing nursing notes"

About: University Hospital Monklands / General Medicine (Wards 2,10,12 and 17)

(as a service user),

I was in Ward 2 of the Monklands in February at 6pm while a auxiliary was about to do my observations they realised my nursing notes where missing and hadn't been seen from 2pm (a student told me this) I was discharged the next day around 6pm. as of 1pm the notes where still missing when I asked.

Noone official told me there missing, I only knew as a staff member looked under my mattress as I lay in the bed, I wasn't told what information was missing (name, address, dob, telephone) never mind all my observations and health details and other data I may not even realise was in it. I don't know if the data protection officer was informed of this breach of data and still to this day I don't know if these notes where ever found, who has them, and members of the public entering the ward had access to them as they hand outside the 4 bedded room for anyone to look at and steal?

Dear Iz84zy,

I am sorry that you had this experience during a recent stay within Ward 2. We treat the security of confidential data as key priority within the site so I'm sorry if you felt this wasn't the case during your stay. If possible it would be helpful for you to share your details and I can look into this further on your behalf? My direct number is 01698 753374 and if you were kind enough to give me a call then we can ensure this is investigated fully.

Regardless I will speak to the ward around the storage of confidential information to ensure we learn from your case and avoid future re-occurrences where possible.

I apologise for any distress this may have caused you and thank you for flagging to us,

Kind Regards

Claire
