My appointment letter had the wrong room on it so I walked in to the start of a private NHS staff members meeting. My Gluten free prescription form had the wrong GP surgery on it.

Thank you for taking the time to send us your comments regarding your recent experience with our service. I was disappointed to hear about your experience and am sorry that there was a mistake regarding the venue for the Group session and that we seemed to have got your GP details wrong on the form for registration with the GFFS.

So that I can look into why these mistakes occurred and to learn from your experience - it would be really helpful to chat on the phone.
