"My 86 year old Dad has been discharged."

About: Roundwood Park Medical Centre

Today, I learned that my 86 year old Dad has been discharged from this surgery because he might need a home visit.

He has been with this practice since it opened and was with the predecessors to this practice for a total of 53 years.

Dads main problem is age and the wear and tear that comes with living a long life, thankfully nothing major.

I have discussed the above with the PM but sadly, just in case Dad needs a HV they have discharged him as he is not in the catchment area.

I am very upset that after such a long time with this practice, it seems that when my Dad, may actually need medical help, advice or assistance he has just been discharged.

This seems to be a practice that caters for young and well people, stay in it while you are BUT when you get ill or old, make sure you move elsewhere because if you don’t...they’ll move you.

We follow the NHS England patient registration guidance on out of area patients. This guidance applies to all patients who are registered outside our catchment area. You can find details of our catchment area here: https://roundwoodparkmedicalcentre.nhs.uk/new-patients/catchment-area/

When considering if it is appropriate to have patients registered who live outside of our catchment area, we will take into account whether the individuals health needs can be safely and effectively provided by a GP practice that is away from their home address.

If it is more clinically appropriate and practical for the patients specific healthcare needs for them to be registered with a GP practice closer to their home - for example so that the practice can provide any home visits or urgent appointments more conveniently for the patient - then we will invite patients to register with a practice nearer to where they live.

We will always give notice of this decision in writing to allow patients or their representatives the time they need to choose a new GP. If patients or their representatives need help finding a local GP practice, they can access the NHS choices website (www.nhs.uk) or telephone NHS 111 who can provide details of local NHS services.

We will always, where possible, transfer the patients medical records electronically to ensure that our GP colleagues at their new practice have the medical history and medication details they need to take over the care of one of our patients quickly and easily.
