"Caring for elderly parent in the home."

We are being encouraged to look after our parents in the home to take the strain off the NHS and local Government resources and I am happy with this and my Mother, and my Father before her, wanted to remain at home for as long as possible. This happened and is happening, (my Father passed away in 2014 and I am now currently full time carer for my Mother) but we were very much left to our own resources for my father and now it seems with my Mother also. If end of life care is to be encouraged in the home then I feel that more should be done by the NHS and local community care teams to enable those individuals (and it often happens to be just one of us! ) to cater for all the physical and emotional needs an ageing and dying parent has. Palliative care professionals receive training for their jobs ... we do not. So, a visit from a Doctor or Nurse when the patient is ..say ... 89 plus... and still at home, for 30 minutes to assess their health and give advice as to how to proceed .. or maybe a forum online set up by the local surgery overseen by a Dr or Nurse in respect of end of life care in the elderly to respond to questions from carers .. this would be a practical help and emotional - for the sole carer to feel they are not alone in this journey which is not only hard physically but emotionally too. I mention my Father ... he died in 2014, I was with him and he died in agony and extreme fear .. it has left me emotionally damaged, (his GP healthcare was minimal and in my opinion quite callous.. in fact, the emergency Dr arrived from another practice at the very last few moments and asked for the end of life care plan only to be met by confusion as I had no idea such a thing existed! )  and I fear the same will happen now with my Mother. We do not exist as far as the local surgery is concerned, even getting a flu jab for her at home is like a huge effort.. I know surgeries are under immense pressure but collating computer data for all those on their lists over - say - 88 and having a twice yearly check up at home? Is that too much to ask for our elderly housebound people who are facing such huge physical and emotional pressures as their near their deaths?
