"85 year old father discharged from Home Ward without any notification left with diabetic inflected ulcers on foot"

About: Ealing Community Partners - Adult Community Services / Ealing Adult Rehabilitation Services (Physiotherapy and Occupational Therapy) Ealing Hospital

My father was recently discharged from Northwick Park Hospital after a month in the hospital, our GP had him referred to Ealing Home Ward whose role ostensibly is to support people who are vulnerable and prevent readmission to a hospital by providing short-term care in the home setting.

My father is a type II diabetic and has a problem with ulcers related to this condition, the home ward team, GP and district nurse were also made fully aware of this as part of his discharge from the hospital.

Despite this I am sorry to say my father has been on the wrong end of what felt like shocking neglect by the Home Ward Team and District Nurse. His diabetic foot has not been looked after and as a result he has been admitted to Ealing Hospital Today. What I am incredulous about is the fact that no one has properly cared for his foot. Worse we also informed that Home Ward had unilaterally and without any notification to my father or our family discharged him from their "care" effectively abandoning him when he was at his most vulnerable. I should also add he has a stage 2 pressure sore that is he has been abandoned to cope with on his own.  I feel this is a safeguarding issue and that it is high time someone whether that be our GP, Homeward or the district nurse take responsibility and ownership of his care. I will be making a formal complaint about the way I feel that my father has been dumped by Homeward.
