NHS inform is Scotland's national health information service; you can find out about your rights and how to access your health records via this link www.nhsinform.scot

In short, you'll need to contact whoever holds the record, so that would be your GP or the medical records department of the hospital(s) you attended.

To access your GP records please contact your GP practice. For NHS Lanarkshire hospital records please write to the Legal Section Medical Records Department Monklands Hospital who can deal with your request. Alternatively if you provide me your email address I can send you the proforma.
