"In limbo due to poor communication"

About: Avenue House

(as a relative),

My mother in law who has bipolar disorder for over 30 years, always has issues with Avenue House.

Currently she needs her medication reviewing, but her GP won't do it as she is a discharged patient of Avenue House.

Avenue House discharge her routinely even when it is obvious she is is still in need of help.

Currently she is in limbo and  rushed to A&E on 2 occasions, she needs help but the communication between her GP and Avenue House takes so long and she suffers as a consequence.

Speed up the communication and reduce the time between referrals.

I am sorry to read that you have been dissatisfied with Avenue House. Without full details of your mother in law's name it is difficult for me to look into the exact nature of your concerns and provide a full response. If she wants to call me at Avenue House I am happy to speak with her.

If your mother in law has been discharged from Avenue House, her GP would need to re refer via SPA. If she has been discharged the care of your mother in law would be under her GP and they would need to review her medication or refer back to secondary care.

Referrals via SPA that are urgent go via the ECATT service and those that are regarded as routine will come back to Avenue House. We have a referrals meeting weekly with a Dr where we consider all of the referrals to the team and new referrals are seen within 2 -3 weeks by either a health care professionals (either a nurse or social worker).

As a carer you can also contact the SPA (as well as your mother in law).
