(as a relative),

My Father who needs a full at home care package will be discharged from hospital at 8am Monday morning. This is a full 9 hours before care is due to begin at 5pm.   The hospital are refusing to make alternative arrangements as it is the weekend saying patient transport is booked for 8am and cant be changed.   Its extremely sad that despite vacant beds, they feel it it acceptable to discharge a frail patient so early.   I fail to understand why they have not collaborated with the care providers, when I thought collaboration between services was the way forward for the NHS.

Dear SAJRJJ,

Thank you for contacting us. The Senior Charge Nurse at Jubilee has been in touch after reading your story and has asked me to post the following message to you:

'Apologies for any distress this has caused yourself and your father regarding an early morning discharge with care support not being on place until 1700. When my colleagues learnt of your concerns they immediately cancelled the patient transport booked for today and rebooked an ambulance for either this afternoon or for tomorrow morning. We will contact you once we have a better indication from the ambulance service of the time that your father will be discharged that coordinates with the care he requires.'

Hopefully this has made things a bit easier for both you and your father.
