What I liked

Treated at all times with care , kindness and consideration.

What could be improved

The discharge procedures were poor and in my view way below acceptable standards. Within an hour or so of arriving home I, a 96 years old lady had fallen and my husband who is 86 years old and severely disabled contacted an elderley neighbour to get me up and into bed.

I am told that in other areas when a vulnerable person is discharged an experienced person would be in attendance to ensure that as reasonably as possible the patient would not be put in harms way. This would seem sensible and cost effective for the NHS.

We understand from Care Direct that our experince was not at all unusual and that it 'happens all the time' if true then someone should be listening and taking appropriate action to improve this important aspect of the care package.
