"Seamless change from hospital to home (Homeward)"

About: Ealing Community Partners - Adult Community Services

(as the patient),

In rehab ward of Ealing Hospital: Visited by S Walker, physiotherapist, pharmacist. Set up a return to home- visited my home, brought me night dresses etc. After 10 days Returned home. Much changed- to enable me to live comfortably on bedroom floor- put my microwave in spare bedroom, supplied Zimmer frame and commode (but commode placed in my bedroom and in a corner I cannot reach). Seamless change from hospital to home. All the carers were punctual, very polite and helpful. Careline installed within 2 days. Regular visits from James and Kerry (Physiotherapy) who gave me confidence to use the stairs so I was able to return to my kitchen and cook my own meals, The only comment on your service I wanted to make: Too many departments- I got confused as to " who does what".

Thank you very much for your positive feedback about the Home ward service following your discharge from hospital. I am pleased that James (the Physiotherapist) and Kerry (the Occupational Therapist) were able to help you achieve your goals and regain your independence.

On the other hand, I appreciate your comments about “too many departments” and not knowing “who does what”. This is very helpful feedback which I will share with colleagues so that in the future teams try to minimise the number of people needing to be involved at the same time and provide clarity over their roles.

I hope this will be less confusing for people for that are involved with the Home ward service in future.
