"Would be helpful to be reminded to register"

About: General practices in Ayrshire & Arran

(as the patient),

I obtained a Patient Access Registration Form. Due to my own negligence, I did not go online for just over 2 weeks, by which time my personal registration details had expired. Reception advised me that I should return to surgery for a new form with fresh details to complete online within 2 weeks of receipt.

If this situation happens regularly, would it be helpful to jog patients into prompt action with the additional qualification ".......to register and create your account online within 2 weeks of receipt, after which time PIN and IDs expire and will require to be reassigned".
