"Find out my interests before discharge"

About: St Andrew's Hospital Womens Pathway

(as a staff member posting for a patient/service user),

I am now discharged. Being in the community I find that you have to find your own activities and it's possible to become isolated. The staff at my place are brilliant and will support me by attending groups etc. with me and they also help me with my problems. I have always wanted to be busy so activities are very important. The activities I mean are the type where you are sign posted to what activities that are going on in the community where you are living. I have had a great team at St Andrew's so it's difficult to say what could be improved, but something to think about is to find out what my interests are before discharge and give me information about what is out there so I can prevent myself form becoming isolated and keep busy.

Thank you for this feedback and I am very pleased that you have successfully managed the transition out of hospital. All staff, including myself, feel really motivated when we hear the positive comments like these and we all want to do our best to carry on delivering great care.

At the same time, we want to keep improving what we do and I take on board your points on paying greater attention to someone's interests in order to support them to pursue these interests constructively once they've left hospital. It can be hard to find activities in the community that match peoples' interests so we will try and develop a better understanding of what is out there to help us do this better.
