I am pleased that the team have been involved in supporting me post discharge as it has been a big adjustment to come home after such a long stay in hospital, it is nice what the team are doing. When I came out of hospital I was so depressed. At points I wanted to die and I was so worried about everything. I didnt know how I'd feel like before again. It's been very useful to not just have someone to take me out but someone to talk to everyday. Its helped me to feel human again. I thought that the service would be intrusive but I have been able to get out and about and be more independent, which has been down to the help of the team.
