"Care my mother received"

About: Intermediate and Transitional Care / Community Discharge Service

(as a relative),

My Mother had a fall in her house which resulted in a hospital stay for a few days. She also suffers with her memory and mobility is not how it used to be.

On release from hospital my mum had a visit from a physio called Matthew. He has provided excellent care and visits to mum over the last few weeks along with the OT Emma.

They have been on the ball with sorting out carers and providing equipment to facilitate mum at home. Myself and family are very appreciative of the help with Mum as we as a family are working full time and are unable to be there to make sure she has her medication and meals.

Excellent service!

Thank you so much for sharing your story with us. I appreciate that, as a family, it is not always possible to do everything and this can cause a lot of worry about our loved ones. I am so glad the input Matthew and Emma have had over the last weeks has been of benefit, and hope that has given some comfort and reassurance around your mum's care.
