"Felt supported and safe in my own home"

About: Adult Community Services / Domicilliary Care Service Enhanced Care / Acute Care at Home Enhanced Care / Intermediate Care Services (ICS)

(as the patient),

I was provided with carers 4 times a day after I came out of hospital to wash, dress, prepare food, give medications and assist me to the toilet. Over several weeks I felt the carers helped me get on the road to recovery as I was unwell on two occasions requiring acute care support.  The co-ordination of care with acute care team, intermediate care team and GP complemented each other to ensure I was managed and kept at home with my husband and family. This was the most important factor as an older person as I felt I was part of decisions, received individualised care and felt supported and safe in my own home.
