About: Clydesdale Locality Support Service / Care at Home General practices in Lanarkshire ICST (Community Nursing, Physiotherapy, Occupational Therapy)

(as a service user),

My elderly mother became acutely ill. She was cared for by St Lukes Medical Practice and NHSL hospital at home team. 

From the call handler at the Medical practice to the Hospital at home team , practice pharmacist and local pharmacy, my mother's care was exceptional and prevented without doubt an admission to hospital. 

Everyone was prompt , honest and professional. 

At no time did we feel like an inconvenience, in fact everyone went above and beyond to provide seamless care . 

Our family are grateful to everyone for their input , the compassion and respect shown to my mother and the support we felt as a family. We were reassured that Mum was being cared for appropriately and more importantly at her home .

Thank you for sharing your experience of St Lukes Medical Practice.

I am so pleased to hear that your mother received excellent care from the medical practice and the practice pharmacist. Open and honest communication with you as a relative can be so reassuring and help you understand your mother's condition and how best she can be supported, as well as providing you with some support that our teams have excellent knowledge and provide compassionate care.

I look forward to sharing your experience with the team who I am sure will get a real boost in hearing this.

Thank you for taking the time to contact us via Care Opinion. We are delighted to hear that we were able to provide your mum with a high standard of care, which she needed to help keep her at home and out of hospital. We work very hard with our GP colleagues and the community teams in Clydesdale to do all we can to work together to provide patients with all the care they need to keep them safe at home. It is lovely to receive your feedback to know that everyone was able to do this.
