A few weeks ago my elderly mother (with cognitive impairment) received 3 extra packs of medication - all incorrect.
I reported this, it was the GPs error. I spoke to the Practice Manager who assured me that it was taken seriously.
I have photographic evidence.
Then, last week, instead of all the meds in her special pouches, she received a separate pack of her blood thinning meds...which were missed from the pouch. Again, GP error.
For a woman with cognitive impairment * this caused huge confusion.
I intend taking this further. But so far there has not been a formal apology.
