(as a service user),

I found both the physiotherapy and occupational therapy personnel who attended to my recovery from the injury incurred from accidental fall I had in house to be both professional and friendly on their visits.

I would like to take this opportunity to thank them all for everything they contributed to in getting me back to my present state.

I'm really happy to hear that the therapy you have received at home following your fall has helped you in your recovery. The community team will be so pleased to hear that they have played such a positive role in your rehabilitation.
