"Admin issue and negatively affected service user "

I had a shared care agreement dated 5th March 2024 for my ADHD medication to be issued with immediate effect by my GP. It transpired there were missing pages to the version sent to the GP. I last had this medication prescribed two months ago, having to go back in every month to the bedale centre to get a script. I don't know how else to make a formal complaint. It involves Bedale Centre but also my GP for sitting on this request for two months it may have been longer if not for my chasing up regularly for them to realise missing pages. I hate making complaints but this is not fair to me at all. My shared care agreement sent has all the pages so why would I assume that the version scanned etc to my GP was any different. For any medication patients running out of their medication is unacceptable as it would be detrimental to their health, let alone two months!
