"Intermediate care team support"

About: Crosshouse Hospital / Rapid Assessment and Care Unit (RAC)

(as a relative),

After a home visit from the dr, she referred my Mum (who has Parkinsons disease) to the intermediate care team. She'd had a recent fall and declining mobility and my Dad and I were struggling to care for her alone at home. 

The care team were in touch the following day to offer their support and an initial assessment of needs.

It was a great relief to know we no longer needed to struggle ourselves without support.
