My father was recently moved to Ward 57 for rehab, whilst awaiting a date for the restart of his package of care at home. He is in his 70s.
The staff shortages resulted in him remaining in bed for nearly 72 hours, and poor levels of hygiene/care/patient dignity.
We were unable to get any information from staff. He was discharged in a chaotic and disorganised manner with inappropriate patient transport.
24 hours later, he was taken by ambulance back to A&E with a serious infection and dehydration - a failed discharge which did not surprise us given our concerns.
"A failed discharge"
