"Very rude receptionist "

About: St Gabriel's Medical Centre

Just move to the area, went to get application form 2 weeks prior from the receptionist how said fill out and bring back ID with address. I did this today and the same receptionist was on duty. I gave my resignation from as new patient and ask for ID and prove of address. I had changed my Driving License to my new address 3 weeks ago. This is a legal ID and address Document. The receptionist now wanted a letter of proof of address which was never said on getting the application form. This was refused and had was told not in a pleasant manner to come back. The customer service was very poor not even sorry or any apology she was vey cold. At this point I ripped up my application form and walked out. This receptionist needs training in customer service. This place I wanted come to when I’m ill.
