"Not Reviewed Previous hospital notes"

The eye specialist on this occasion lacked expierence due to the previous eye operation . Did not refer to notes and I was not asked appropriate questions . I suffer from extreme claustrophobia and a cloth was placed over my face which resulted in a panic attack and I suffered trauma after the operation.
