"My poor mother was beside herself with worry all day"

I recently attended Antrim A&E with my elderly mother who is hard of hearing. She had been told to attend by her GP to get an abscess drained which was under her armpit. The GP had provided a letter for the hospital. She went through triage and was signposted to minors.

We sat there for 6 hours without seeing anyone. She was then called and examined by 2 doctors who said yes just needs drained (we knew that already) but that they couldn't do it and would have to page the surgical team!

We went out into the corridor (a crowded corridor) no seating left, people laying on the floor, some vomiting around us, lots of people having to stand with drips attached. We waited a further 2.5 hours before being called up to EMSU.

There she waited a further hour to be seen by a surgeon, who again examined her and said yes it was a simple drain and asked their colleague to do it. They went to get a nurse and by this stage it was handover and they said they had to wait for the nurse to come back. When the nurse came back an hour later, guess what - no doctor and more waiting.

Finally the less than 5 mins procedure was completed and my mum's wound was dressed. I finally thought we were going to get home (we had nothing to eat or drink since the morning) when I was told that we had to wait for another Dr to write her letter for the GP. This came back with a request for antibiotics - they didn't have enough of this particular type to give to my mum so a new letter stating a different medicate had to be written. This took another 2.5hours.

We eventually left the hospital last night after 12 hours! This was completely unacceptable and in my view the whole system is broken. I do not understand why general practice could not perform this minor procedure, and I definitely cannot understand why Dr's in A&E cannot do it. My poor mother was beside herself with worry all day.

I am sorry that your mum had to wait to be seen in the ED. The department has been under pressure for a number of weeks due to the acuity and volume of patients attending. Minor surgical procedures are not undertaken by ED doctors nor are they done in the ED hence the reason the surgeons were contacted.

Thank you Gerry for your response. This further supports my view that the current system for minor surgical procedures is not fit for use. If an ED doctor was never going to be able to undertake a minor surgical procedure why did my elderly mother have to wait over 6 hours to be seen by one? She had a letter from GP with diagnosis and did not require further testing. I am struggling to understand the pathway here. Surely there should be something like EMSU set up for minors to avoid backlog in ED?

Hi Katfk54

My name is Helen Davis and I work as an Advanced Nurse Practitioner in our emergency surgical unit (EMSU). Our unit accepts referrals from GP's or ED for patients with an acute surgical condition 24 hours a day.

I'm sorry to hear you mum had a significant wait until she was able to get home. We have previously communicated to our local GP's that our emsu service is available for direct access should GP's contact us to refer the patients. This would have directed your mum to us and would have resulted in an appointment earlier in the day.

I am aware that there can be delays after 5pm when we have less staff on. In particular if we have patients requiring surgery our team can be delayed for some time, we are looking at this by extending our ANP cover to help avoid delays to patients

I hope your mum is recovering well. I will ask our communication team to recirculate our referral criteria to local GP's again.

Just wanted to update you to say I have discussed with my director and he will re share our EMSU pathway with the GP's at next GP forum which is at the end of this month.
