About: Adult Services / Condition Management Programme

(as the patient),

I was referred to the Condition Management Program by my Physical Therapist. I have been suffering with chronic pain and back issues for the past 2 years which seemed to be getting worse. At this stage I was being sent from pillar to post to different medical professionals who I felt were just going through the motions, which also affected my mental health.

When I got my phone call from Caoimhe I felt straight away that she really cared and had my best interest at heart. From my first appointment until the end these people really did their best for me. I will forever be grateful to Caoimhe, Natalie and Raymond for everything they did for me. It was a really sad day when my time had ended with these people. I cannot recommend this program enough for anyone suffering from not only pain but any condition and who feels as though they don’t know where to get help or who can help them. 

Thank you folks for all your help.

As the Coordinator of the Condition Management Programme (CMP) I am pleased to learn that working along with our multidisciplinary team helped you to cope with pain you suffer with.

Within the Condition Management Programme we take a holistic approach looking at the impact of a condition on both your physical and mental health. You described how the chronic pain and back issues you experienced also affected your mental health. The staff you worked with Caoimhe - Physiotherapist, Natalie - Occupational Therapist and Raymond -Specialist Mental Health Nurse/ Cognitive Behavioural Therapist will be delighted to receive this feedback knowing that working together using their respective skills enabled you to improve. Thank you for taking the time to share your positive experience of participating in the programme feedback, knowing that you felt cared for, understood and that everyone involved in your care did their best is heartening.

I welcome your suggestion re advertising and thought it may be useful to share that we are currently working alongside the Communications Team in the Western Trust on a range of marketing and promotional activity to advertise the CMP to a broader audience.

Referral to CMP has become much easier in recent years, and anyone interested can contact CMP and refer themselves directly, or referrals can come from other Health Care professionals such as the G.P. or in your case the Physiotherapist in addition to Work Coaches or other Employment programmes.
