Referred by ambulance team - had been lying on floor for 7 hours.
SW Frailty support team- Avon valley - all wonderful!
Took bloods - requested medication from GP.
Found noise on lungs , requested x-ray, build up of fluid.
Referred to consultant at Bournemouth hospital. Frailty team were spot on!
They arranged equipment and really got on top of everything.
Excellent team!
