About: South West & Southampton Division / Frailty Support Team - Lymington

(as a staff member posting for a carer/relative),

Referred to frailty team by GP after recent hospital discharge.

Everyone that has visited has been very helpful.

We have benefitted from the information given to us.

Very pleased with the service.
