"My Nan's heart attack"

(as a relative),

My nan originally went into hospital for dehydration from gastroenteritis and while she was in hospital she suffered a heart attack and was transformed to another hospital that could deal with here. When she was released the community team listened to my nan and myself (granddaughter/carer) they looked after her and treated her very well and sat and talked to her and did all the personal stuff. I could not of asked for better. Thank you 😁

Thank you so much for taking the time to review our Community Therapy services. I am sorry to hear that your Nan had been unwell.

Our Discharge to Assess teams are responsible for supporting people out of hospital after a period of illness. I am pleased to hear that you grandparents felt listened to and treated well. We recognise that coming home from a hospital stay can be difficult and we aim to work with our patients and families to understand what their needs are and how we can best help and support.

I will be sure to pass on your kind words to our staff members.
