Story by daughter of female patient:

"The Acute Care at Home team came in to see my mum and checked all her obs, done heart trace and everything.  They always told me what was going on and were very friendly and made sure mum was well when she was signed off, usually phoned later the following week to check in how mum is doing.  Carla the pharmacist always kept me informed and phoned me on a few occasions to keep me updated about mum's meds.  Very happy with the service provided."

Thanks for your feedback.

The team respond to referrals within a 2 hours period. On admission we complete a full head to toe assessment including bloods, heart tracing and medical review and book tests if required.

Carla also completes a full review on each patients medication and links with patients and family to identify if certain medications are no longer required and explain new medications.

The staff visit and give deliver treatment plan as prescribed. I will pass on your feedback to all the staff.
