Story from patient:

"The team were very courteous, professional and extremely helpful and I would highly recommend this fantastic service.  I was also advised to stop certain medications by the Pharmacist and they contacted me with advice of the way forward."

Thanks for your feedback.

We work as a MDT on admission and involve what members are required.

The pharmacist plays a key role with a patients reviewing all medications.

Medications are explained to patients and written details of what to stop is given out also.

Its great to hear you found all staff polite and professional on each visit.

I will pass on your feedback to all the staff.
