A relative was discharged from Blackpool Teaching hospital last week after a 7 day stay as a result of suffering a stroke.

On discharge my relative was given a big plastic bag full of drugs and given no instructions for administration. The bag included 2 packs of warfarin which had no instructions with them. It turns out after talking to the GP that the warfarin was not to be taken until he gets an appointment at the anticoagulant clinic.

My relative was confused and very frightened to be given such a lot of medication with no information on what to take and when.

He was also told he would get a community visit from an occupational therapist to assess living arrangements at home and that has not happened. He has subsequently been readmitted to hospital 3 days after discharge with D&V which could possible be a hospital inquired infection he developed during the initial hospital stay.
