Here's an example of inefficiency in the NHS.

On the Saturday I was puzzled by a letter I received by first class post cancelling an outpatient's appointment- which I hadn't got! Then on the Monday I received, by second class post, the letter giving me the appointment!

Why make an appointment and immediately cancel it? What a waste of time and postage.

I then had to go to my GPs to find out what was going on, and get the results of a scan.

I agree this does indicate inefficiency. If you might contact me directly on 0115 969 1169 ext 56696 we can look into this further to check this is a one-off as opposed to a wider problem.
