I was admitted to the hospital as an emergency because I collapsed I stayed in overnight in the overflow and observation ward if they can treat you there they do if they cant they admit you to a ward if it’s more serious.

when I was told that I could go home it was late afternoon so I asked what happens now and I was " discharged" without any paperwork or medication to take to my GP which I was surprised about.

the hospital said they would write to my GP direct to have me referred to a neurologist but I was unclear about the procedure I was allowed to leave the ward but I was not feeling very well I attempted to leave but did not feel able to use public transport and I re-entered the departure room and spoke to a receptionist and asked if she could help me by ringing for a taxi to get me home, she seemed surprised and asked me were I had come from I told her I had been in the overnight observation ward and I did not have any discharge papers which she had asked me for. I was unable to give her the paper as I did not have any when I was told I could go home so I showed her my hospital tag.

I saw my GP yesterday 19th April and he advised he had not got anything from the hospital the GP cannot refer me until he is contacted direct from the hospital.

I rang the hospital this morning 20/04/2013 and spoke to the A & E reception who informed the GP can fax over a request I thought the hospital would automatically contact my GP.

Now I have to request my surgery in writing to ask them to fax over a request to the hospital for me.

I feel the discharge procedure is not made very clear before they are released from the hospital
